Potential MRI NSF - 71 yr old male

Client does not have any known kidney or liver problems but he had 1 MRI with contrast and 1 Angiogram with contrast done in September 2004 or September 2005. Since these procedures, client has developed red/dark patches of skin, rashes, itching, flaky/bumpy skin, joint pain/stiffness, hip pain, rib pain, muscle weakness, muscle "twitches" in his thighs.

Potential MRI NSF Client 45 yr old Female

Client does not have any known kidney or liver problems. She had 1 MRI with contrast done in November 2008. Since the MRI ,client has developed red/dark patches of skin (elbows/back/leg), rashes, burning/itching, bumpy/scaly skin, swelling/tightening, joint pain/stiffness, rib pain, and muscle contractures.

Potential MRI / NSF Client  55 y/o male

Client was diagnosed with diabetes 2-3 years ago.  He does not have any known kidney or liver problems.He has had several MRIs with contrast.  The last one done 2-3 years ago was with contrast.He suffers from red patches of skin, hives, burning/itching, scaly skin, swelling/tightening, hardening, joint pain/stiffness, hip pain, rib pain, muscle weakness and muscle contractures.
